Welcome New Patients!
Before your first visit, we will e-mail you a web-based intake form from our secure patient portal. Once completed, your patient data may be imported automatically into our system. This intake form may be done on a home computer or with the use of an iPad or touch screen computer.
To save time at your Initial Consultation, you may print and fill out the set of new patient forms available on our Forms page. If you don’t have a printer, no problem! Come fifteen or so minutes early to your appointment. We have plenty of copies at the office for you to fill out. Completed forms may be faxed to 317-872-9303 or printed and delivered in person.
WHAT TO EXPECT AT THE FIRST VISIT
NEW PATIENT PAPERWORK
Above is a link to the new patient paperwork. Click and print off the paperwork and bring it in with you to your first visit. This will help expedite your first visit, since the first visit can usually take about an hour and a half. If you do not have a printer then you can come to the office and fill the paperwork out here.
During your consultation, the doctor will discuss your needs in greater detail and share with you what our clinic has to offer. You will then receive a complete examination. This will include the following painless procedures: two neurological scans, reflex and range-of-motion testing, and standard orthopedic and postural tests. Lastly, a pH saliva test will be collected to determine the alkalinity level of your body. To ensure the accuracy of your pH test, please do not eat or drink anything except water for two hours prior to your appointment time.
No unnecessary tests will be performed. All tests we administer are necessary to the complete understanding of what is involved with your case and will assist your doctor in prescribing a complete wellness program.
One of our skilled doctors will perform a detailed exam including: postural assessment, range of motion, neurological tests, orthopedic tests, and palpation of the area to determine what tissues are affected and to what extent. Your consulting doctor will then be able to explain the findings to you to help you better understand what is going on and the necessary steps to further confirm the diagnosis. We will then explain all treatment procedures that deem fit for your condition. You may ask any questions you have at any time!
Your condition may require radiographs to either rule in or rule out more serious conditions, or better evaluate the structures to help develop the most effective treatment. If you have had X-rays taken within the last year prior to visiting our office, we ask you to bring them with you.
Treatment on your first visit is not common in our office because the doctor always takes the appropriate amount of time to review your history, examination, and imaging. All treatments and pricing schedules will be thoroughly explained to you before beginning the process.
REport OF FINDINGS
A follow up visit will be scheduled within days of their initial visit to discuss X-ray finding, treatment options, home care exercises, and a recommended treatment plan.
